Impact Profile: Solera Health

When the National Diabetes Prevention Program (DPP) became covered under the Affordable Care Act, it brought DPP to millions of at-risk Americans, and brought a mountain of work to healthcare providers newly eligible to be compensated by health plans. Patient identification, referrals, electronic claims, auditing, fraud prevention, and compliance loomed as prohibitive undertakings for small providers.

Solera was created as a technology platform that links healthcare providers to patients and their health plans. Solera handles the “back office” chores of healthcare providers, acting as a single point of contact to Medicare and other health plans, which allows providers to focus on what they do best: provide care.
